I'm going to try to keep this as short and to the point.as I can I have 3 herniated lumbar discs, My insurance carrier recommended and required me to have Epidural Steroid Injections. I declined, they forced me to or else they would not pay for any further treatments.
The first ESI done by anesthesiologist in hospital had caused a dural puncture, punctured my spinal cord, this resulted in nerve damage and furthering my disability.
My Insurance company insisted and required I have 2 more ESIs

My employer has a third party administrator for our life insurance plan-basically they collect our premium and pay it to the carrier. In October, 2009 our life insurance changed to another carrier-not by our request. We had an employee on long-term disability before the change. We have a waiver of premium clause; however for reasons undetermined it was never applied for. My employer paid the premiums for this individual for the life of this employee-first to company #1 and then to company # 2

Workmen's compensation carriers have the right to investigate for 90 prior to denying or accepting a claim.

The insurance carrier says permanent stationary disability. My attorney says I will probably be eligible for vocational rehabilitation. So what are my options. late last year, President bush passed a bill that diabetes is a disability. It was written in one of the diabetes magazines. Yet, I can't get health insurance. When I find a carrier who will insure me, the price is so high, I can't afford it and my diabetes supplies. Without the insurance I can't afford the supplies. My insurance company is telling me that I need to re-pay them money. Not sure yet how much. I have been on LTD for 3 years. Just got approved for social security but for a closed period only. I received a back check for 3 years of S.S. I don't understand why you would have to pay back something that you paid for every week in your paycheck? Would appreciate any comments.
